Either the forklift wasn't maintained by the company your dad worked for and it is their fault, the work area was not maintained so it was unstable and it is the property owner's fault, or your dad was operating the forklift incorrectly and it actually was his fault. The point of these, is that workplace accidents can be avoided if everybody does their job, not just the person affected by the workplace related incident.

After reading many of the comments, I realized that viewers are more or less affected based on the experiences in their lives, example, losing a loved one in a work place accident versus being the supervisor who overlooked a faulty shelf. The point is that safety is about teamwork, it is your employers responsibility to provide you with a safe environment and proper equipment. However it is your choice to protect yourself by being cautious and using the proper equipment to do the job safely.

The only thing worse than having all those things you described happening to a person is to know you a child, parent, relative, friend, or even talking to you. Yes, you are so annoying and stupid that having hot grease melt away your face is the better choice between that and having a conversation with you.
